To examine the ethical issues involved in governmental decisions with potential health risks, we review the history of the decision to raise the interurban speed limit in Israel in light of its impact on road death and injury. In 1993, the Israeli Ministry of Transportation initiated an “experiment” to raise the interurban speed limit from 90 to 100 kph. The “experiment” did not include a protocol and did not specify cut-off points for early termination in the case of adverse results. (...) After the raise in the speed limit, the death toll on interurban roads rose as a result of a sudden increase in speeds and case fatality rates. The committee's decision is a case study in unfettered human experimentation and public health risks when the setting is non-medical and lacks a defined ethical framework. The case study states the case for extending Helsinki type safeguards to experimentation in non-medical settings. (shrink)

Background: Since the passage of the Patient Self-Determination Act, numerous policy mandates and institutional measures have been implemented. It is unknown to what extent those measures have affected end-of-life care, particularly with regard to the do-not-resuscitate order.Methods: Retrospective cohort study to assess associations of the frequency and timing of DNR orders with advance directive status, patient demographics, physician’s specialty and extent of documentation of discussion on end-of-life care.Results: DNR orders were more frequent for patients on a medical service than on (...) a surgical service and were made earlier in the hospital stay for medicine than for surgical patients . 22.18% of all patients had some form of an advance directive in their chart, yet this variable had no impact on the frequency or timing of DNR ordering. Documentation of DNR discussion was significantly associated with the frequency of DNR orders and the time from DNR to death .Conclusions: The physician’s specialty continues to have a significant impact on the frequency and timing of DNR orders, while advance directive status still has no measurable impact. Additionally, documentation of end-of-life discussions is significantly associated with varying DNR ordering rates and timing. (shrink)

Four models of forgiveness are identified; the health model, the philosophical model, the Christian model and the prosocial model. All define the term ‘forgiveness’ in a way which is consistent with their particular perspective. The authors offer a definition of forgiveness and propose an integrated model of forgiveness which seeks to incorporate contributions from all four areas, but is not biased towards any one model. Four levels of transgression are identified and categorized according to the degree of perceived damage. Apology-automatic (...) and apology-dependent responses are distinguished from a forgiveness response which is restricted to Levels 3 and 4. The advantages of adopting a forgiveness strategy for both the forgiver and forgiven as opposed to other responses, i.e., revenge, denial, and condoning are discussed. In this paper I delineate the group of activities concernedwith business and then proceed to give an exposition of the concepts usedby Ubuntu as an example of the African view of business managementindicating those activities of human performances regarded by them asbasic to their world view. I proceed to deal with the way these Ubuntuconcepts are applied to business management using the ideas of LovemoreMbigi as an important advocate of the Ubuntu style of participatorymanagement. In doing so. I try to (...) establish what some important Africanthinkers themselves regard as the special position of Ubuntu in contrastto individualism, collectivism and Western Humanism. I then focus on themost important topic of this paper, to wit, the application of the Ubuntuconcepts to a real case and analyse it as a case study. Doctors who entered the National Health Service to practice medicine now find themselves forced to practise selection. It seems that patients are being lost at GP level. Surely the basis of a good relationship between doctor and patient relies on trust and trust is based on truth which should not be concealed from patients. And should any one dare decide the quality of life for another human being?

Genetics and genetic testing raise key issues for insurance and employment. Governmental and public concern galvanised the British insurance industry into developing a code of practice. The history of the development of the code, issues of genetic discrimination, access to medical information, consent and the dangers of withholding information and the impact on the equity of pooled risk are explored. Proactive steps by the Association of British Insurers suggest that moral reflection not legislation is the way forward.

I have tried to focus on issues which have not been customarily dealt with or emphasized in the scholarship on Kierkegaard with the exception of the writings of David Swenson and (...) Paul L. Holmer to which (and to whom) I am greatly indebted. Some of the positions for which I have argued in this volume (especially in Chapters IV and V) may be controversial. Measuring event-related potentials (ERPs) has been fundamental to our understanding of how language is encoded in the brain. One particular ERP response, the N400 response, has been especially influential as an index of lexical and semantic processing. However, there remains a lack of consensus on the interpretation of this component. Resolving this issue has important consequences for neural models of language comprehension. Here we show that evidence bearing on where the N400 response is generated provides key insights into what it (...) reflects. A neuroanatomical model of semantic processing is used as a guide to interpret the pattern of activated regions in functional MRI, magnetoencephalography and intracranial recordings that are associated with contextual semantic manipulations that lead to N400 effects. A phenomenalist philosophy which employs the Principle of Acquaintance (PA) plus the Principle that what exists are the referents of certain meaningful terms, defined by PA, cannot include either universals or particulars in its ontology, but is limited to instances of universals as constituting the range of ontological existents. Universals must be omitted since they are repeatable and, hence, never wholly presented or contained, whereas the objects of direct acquaintance are wholly and exhaustively presented. Furthermore, no entities beyond characters (qualities (...) and relations) are given in direct acquaintance; hence, particulars, too, must be omitted for the phenomenalist who relies on PA. (shrink)

We examine the evolution of the concept of stability in community ecology, arguing that biologists have moved from an emphasis on biotic communities characterized by static balance, to one of dynamic balance (returning to equilibrium after perturbation), to the current concept of stability as persistence. Using Wimsatt's (1987) analysis of how false models can often lead to better ones, we argue that failed attempts to link complexity with stability have significant heuristic value for community ecologists. Nevertheless, we argue that, (A) (...) because there is no common characteristic that stability terms presuppose, community ecology might be better served by abandoning the concept of stability and by employing instead specific terms such as 'persistence', 'resistance', and 'variability'. (B) The current emphasis (of stability terms) on persistence of species provides little basis for explaining possible mechanisms that might account for persistence. (shrink)

In Reason and Analysis, Prof. Brand Blanshard criticises the logical empiricist view regarding necessary statements, including the laws of logic. He distinguishes four component these of this view: (I) Necessary statements--here, the laws of logic--are resolutions or reports of linguistic usage. (II) They are conventions. (III) They are analytic (tautologies). (IV) They say nothing about the world. In this paper I first show that Prof. Blanshard is essentially right in his criticisms of (I), (II), and (IV); but that he has (...) done nothing to show that (III) is incorrect with regard to the laws of logic. Then, second, I attempt to show that even though (I), (II), and (more importantly) (IV) are false, (III) is true. That is, the laws of logic are tautologies, and yet, in a sense to be distinguished, they say something about the world. (shrink)
